Company-paid disability

Wife was on disability for shoulder surgery between Thanksgiving and New Years. The insurance was paid solely by the employer, therefore taxable. She received payments directly from the insurance company, which also deducted FICA, but not Medicare. Questions are:

1) is the amount of insurance payments typically included in the W-2 issued by the company? Her W-2 has no special codes indicating this. 2) If not, will we be getting some sort of tax form from the insurance company? We have the check stubs for the payments, is this sufficient documentation for adding the amount on the 1040?
