Crafting a 204(h) Notice to Self

I have a Money Purchase Plan that I want to terminate, like thousands of others have done after EGTRRA, to consolidate into an existing Profit Sharing Plan.



I have read many articles on the required 204(h) notice, including the treatise at this URL:

I am the employer corporation's chairman/ceo/secretary/janitor, and the trustee, plan administrator, and only participant of the plan.



What is the proper way to write a notice to myself to comply with 204(h)? Do I need to hire a lawyer to do it?



How do I even start... e.g. "I hereby notify myself of the following.. please read this information carefully.. and ask yourself if you have any questions"... !?!
