FICA taxes failed to pay - how do I pay now?

My sister was using Quickbooks to do her payroll taxes and to keep things simple let's just say she had no idea what she was doing.



The federal withholding for her employees whe was doing property sending in her 940 and 941 forms and with held funds.



However, on the medicare/ss fica withholding she wasn't sending in the checks or any forms. So, I have a pile of 2 years worth of checks here and no forms.



How can I back pay these? I have the checks for the proper amounts but I have no forms. I don't know who or where to send the checks. What forms do I use and where do I send them???


Something's wrong with the story. Take a look at the 941's, which is where the FICA/Medicare taxes are calculated and paid. If the returns were done properly and the full amount due paid she doesn't owe anything. If they weren't prepared properly amend them on Form 941-X.
