That is the correct form. However, you closed on your purchase several months too soon to qualify for the the credit available to new home purchasers who were long-time resident of the same main home. The earliest closing date to qualify for that credit was November 8, 2009.

The instructions seem to indicate after November 6, which means November 7 or later.
