How to correct an incomplete Form SS-8?

As a worker, I mailed a Form SS-8, Determination of Worker Status for Purposes of Federal Employment Taxes and Income Tax Withholding, to the IRS last week. After mailing, I noticed that I listed incomplete addresses for both myself and the company involved, leaving off the city/state/zip info.



Do I send a corrected SS-8 with a cover letter, or wait for the IRS to contact me (I did include my phone number, and they could presumably find my city/state/zip using my SSN)? Or is there a third course of action?

As long as your SSN and the EIN of the potential employer were present, and match the other entity information given, I wouldn't worry about it.

I don't have the employer's EIN and thus was unable to provide it on the form. (The employer pays through direct deposit only. It will not issue a W-2. No 1099 either - I earned less than $600 from the firm last year.)
