Massachusetts tax question

I need to amend my 2003 return because $1564 of mortgage interest was entered as interest/dividend income. Does anybody know which form I have to use? Thanks in advance.

Form 1040X but it will have to be before 4/17/2007 as the

2003 tax year closes on the 2006 filing deadline (unless 2003 was filed under an extension?).
