what form number is for earned income?

hello, I was told that there's a form you mail to the IRS and with that form, they tell you the amount they expect you to file for tax returns. I think it was called earned income or something like that.

Does anyone recall what form this is?

We tried looking on the IRS website but it's TOO confusing.

I found the note, I think that form was called personal income verification. Or some sort of income verification. Sorry about being vague, too many forms names!
