Does any one using RMS, charges the customers for Fuel Surcharge. If so, is it created as a new Item or added to a separate Freight calculation? Any info is very much appreciated.

You could do it with a non-inventory item called "Fuel Surcharge", or otherwise use the shipping feature. There's only one shipping charge option, so if you charge for shipping plus a fuel surcharge, and want to show it separately, then you will want to use a non-inventory item.
Also note that you cannot apply tax to the shipping charge feature in RMS. In some places you have to charge tax on shipping, so then you must also use a non-inventory item for your shipping charges.
