ESPP Purchace Date/Cost/Basis

This past year, I participated in my company's ESPP every quarter. Towards the end of 2008, I sold all of the stock that I'd bought during each quarter. Now that it's time to fill out my short-term capital gains forms, I'm having trouble figuring out what to put for my purchase date, and cost/basis because I'd been making purchases (at different prices, obviously) throughout the year, since my stock sale is listed on a single row despite the fact that it represents multiple purchases.



Any help here would be extremely useful. (Sorry if this is a dumb question...)

See the Schedule D instructions. You use "various" as the purchase date and add up your investments. If there was a bargain element taxed as wages you add that amount to the amount you actually paid.

You made a disqualifying disposition of ESPP stock. There is some information on the Fairmark site here:
