How to report this income?

I am a licensed dentist. I worked for two days at a dental office, as a temp, and the office paid me directly with a check for $800. They did not take any money out for taxes. Were they supposed to supply a W-2, or a 1099-MISC? Or can I report this income some other way? And if so, in what way? Was I considered a sub-contractor? Self- employment?

Sure sounds as if you are an independent contractor. Report on Schedule C and if schedule C profit is over $433, also use schedule SE.

Yes, but even if they didn't give you a 1099-MISC you are supposed to report the income.

All the same. If you have no expenses you can put it in line 20 of the Form

1040. Or you can more appropriately put it on a Schedule C along with any expenses. In either case the net profit after any expenses is subject to self-employment taxes.

No. Sounds as if they've treated you as an independent contractor, but "temp workers" are often employees. Before going with the above, grab an IRS form SS-8 and answer the questions. Regardless, you'll still be responsible for your half of the FICA taxes.
