making a contribution to HSA to reach max limit for 2010

my HSA allows, based on family enrolled in high deductible health plan, maximum contribution of 6000.00 for tax year 2010.



normally, an automatic pre-tax payroll deduction is terminated at this time of year, so as to avoid going over the limit and this leaves me with a small gap of about



250.00 which I planned on making myself from my after-tax funds.

since I will be paying with after-tax funds, what is the form or procedure that I would use to report to IRS that I made this 250.00 contribution and did so from after-tax funds?
