Reverse Exchange and Filling out 8824

I have a reverse exhange situation....

1. I am doing a REVERSE 1031 Exchange
2. Replacement property is held by Exchange Accomodation Title holder on 6/20/2006
3. Relinquished property is identified 7/10/2006
4. Actual exchange of properties occurs on 8/15/2006

What dates do I put on Form 8824 lines 4, 5, and 6?

The language and instructions point to a straight exchange and not to reverse exchange and so I am puzzled. Any help will be appreciated. Smith
