Life Insurance & Smokers

It's pretty much common knowledge that all insurance companies will charge higher life insurance premiums for smokers. Most I've had a quick look at seem to be double for smokers vs non-smokers based on usually on the lines of one very basic question like "Have you smoked tobacco products in the previous 12 months?"

Is anyone aware of any that will ask for more information as to how much and what format an individual smokes taking that into consideration in calculating their premium?

AKAIK they all ask how much and what you smoke, once you've told them you are a smoker. If you smoke 20 cigarettes a day you'll probably get normal smoker's rates, but if you smoke 100 a day they'll probably increase the premium. Without asking you, they won't know.

Hmm, I wonder - does that mean that if you had, say, smoked one cigar sometime that year that you would then end up paying higher premiums? Also, with that particular question, would someone who smoked 20 a day for 20 years, but stopped a few years ago pay the same as someone who had never smoked ?

Do companies that charge higher premiums for smokers' life insurance also offer higher annuity rates to smokers for the same reason?
