Not sure what you are wanting to report. For income and expenses it depends on the business type. Use:

schedule C (form 1040) for sole proprietorships and single member LLCs, IRS form 1065 for partnerships and multi-member LLCs, IRS form 1120 for corporations, IRS form 1120S for s-corporations and IRS form 990 for non-profit organizations.

Schedule C with form 1040 is due by April 15th. Form 1065 is due on the

15th day of the 4th month following the partnership's fiscal year end. Forms 1120 and 1120S are due on the 15th day of the 3rd month following the corporation's fiscal year end. Form 990 is due on the 15th of the 5th month following the organizations fiscal year end.

Additionally you will need to file a state income tax return appropriate to your business type for all states in which your company did business (where applicable). In addition some states may be require you to file a franchise & excise tax return.

In some cases a corporation or LLC may be required to file the appropriate forms for the state in which it was formed even if it did not due business there. One example is Georgia which has no income tax for s-corporations, but charges a net-worth tax (form 600S).

If you have a partnership or multi-member LLC each partner or member will need to report the amounts on their form K-1 on schedule C with form 1040 by April 15th. If you have an s-corporation each shareholder will need to report the amounts on their form K-1 on schedule E with form 1040 by April 15th.

If you are wanting to report something other than income and expenses write back with more details so I can tell you what to file and by when.
