I can't find an IRS form to list the items and amounts included on line 19 of the 1120S Income Tax Return.

There is no such form. You just prepare your own statement listing the descriptions and amounts and attach it to the return.

I don't think there is a form, I have always just created a Schedule which is a list of items and amounts, and a total that goes on line 19.
Thanks, I guess it just wasn't me not finding it. I've always just put in a typed up list but I happened to see an 1120S prepared by a CPA which had a list which looked so much like the other 1120S pages that I thought it was a real IRS form. I'm guessing he was using software which created the list on that "form".
