A neighbor of mine, an elderly woman living solely on a small SS check and occasional bags of groceries provided by her church, has not filed an income tax return for years - perhaps decades. She asked me to get her the form to fill out to get the rebate she has read about or heard about. So, my question: What is the simplest IRS form for her to use to get on the list of those to receive a rebate? (Rebate here meaning the widely reported $300-1,200 rebate to be distributed this late spring or summer.)

You could e-file it for her through the IRS FreeFile program if you want. I recommend TurboTax.

I didn't know that existed; that is great! How do you handle W2s? Don't they have to be attached?
Nothing has to be attached. IRS verifies W-2s using the employer identification number (EIN). That's why it's important to enter all the W-2 information correctly. If you sign the return electronically, no paper needs to be sent to IRS, which is exactly how IRS prefers things.
