MA tax/Health Insurance

My son lives in MA and is working on his state tax return. The tax return requires him to list who he has his health insurance with. Well, his insurance is changing on 4/1. Does he give the insurance on the date he files (the old insurance) or on 4/15 (the new insurance)?

This is the 2011 tax return. MA wants to know the insurer(s) who covered him from 1/1/2011 to 12/31/2011.

He's working on his 2011 return, right? So he should list his 2011 insurer.

He should have received a form MA 1099-HC from the relevant insurer, it has the information he needs to put on his return.

Okay, just one insurer in 2011, so he is set there; but what about 2012? The second insurer? He didn't get a MA 1099-HC because his insurer is in NY and is unaware he is in MA. (unless they are bright enough to notice that all of his bills are from MA)

Schedule HC has room to enter two health insurance providers on line 4f (and two for your spouse on 4g). If you have more than 2, you fill out Schedule HC-CS.

He should call the insurance company and ask them to issue one for him
