You have to fill in two VAT returns.
VAT registrations cover all the activities of an entity. If you really mean John Smith, one person (entity) with two separate UK businesses, then only one registration is possible and it covers all the business activities of John Smith.

However I believe that Ltd. companies can of course be VAT registered as they are separate entities. So if John Smith set up a Ltd. company called John Smith Ltd. then that will (must?) have a separate VAT registration.
