payroll to non-exist employee

if I found that some payroll checks are paid to someone who is not in the employee file, what could have happened?

What I can think of is

  1. the employee is new and still have not been input into the employee file by the hiring department.(is this possible?)
  2. someone in the company is making fraud payment. Are these two reason correct? Could you think of more possible situation? Thanks.

Keeping in mind that on any given day 1 business in 4 is being embezzled, I'd look there first. Here are some ideas where to look: Who interviewed the employee? Who hired the employee? Who set the employee up in the payroll software? Who authorized the timesheet? What is the mailing address or who received the pay check and delivered it to the "employee?" What is the payroll software you are using? Does it track who did the payroll? It's not QuickBooks is it?

This is not a homework. Just a practice for the use of the software ACL. This is also shown in the book for ACL. But I want to make sure what I am thinking is correct.
