We seeem to be losing transactions that are put on hold from time to time - has anyone else come accross this?
I thought might be just a cashier mistake the first couple of times, but it has happened several times now!

Try re-indexing and checking the database.
SO Admin | File | Connect | Database | Backup | Database | Re-index | Database | Check Do this on the main database machine.
