I am trying to write an addon using the InitialiseTransaction Hook, but for every transaction the hook seems to be firing twice.
Has anyone else had this same problem

Yes. Never found a solution, either.
If you come up with something, please let me know.

Check that there is any trigger that is related to the transaction table, you may have one or more triggers that dependencies to the transaction table.
