I need an account statement modified. I have it partially modified and need some additional changes that I do not know how to do. Is there anyone who can help me out with this.

I would provide you with what I have done, in addition I need
- Please detach and enclose top portion moved down and break line put in correct position under that.
- The line with Credit available removed
- Remove the spaces between each line in the activity body.
- Add the PO (comment) line in the activity body.
