POS Crashing due to app running twice

I run into this problem on occasion when I'm working on a customers POS system. It is not a user error; it's happened to me and I know for a fact I didn't double/tripple click, because I used the keyboard to press enter on the icon.

It is a POS issue, but nobody at Microsoft has been able to reproduce it. I believe it's related to OPOS drivers, because it never happens on my office workstation, but it has happened a few times on my demo machine which has a printer and pole displayer attached. Maybe one of these days when I think of it I'll run procmon on SOPOSUSER.EXE when it happens.
