I was wondering if someone would be able to help me in filling up this form. I am a foreign person owning a US corporation and am trying to open a bank account for my business with a US bank. They have asked me to fill up the W-8BEN form but I am a bit confused on how to. These are: Line 1 - Name of Individual or Organization - Should I be filling in the name of my US corporation or do I fill in my name itself? Line 2 - Country of incorporation - Do I fill in as US or leave as 'Not Applicable'? Line 3 - Similar to Line 1 - Should I choose Individual or Corporation?
Line 4 - Permanent residence - Should I be entering the Address of Country where I have citizenship or if I have been working in a different country (where I was born too), would this address be possible? My citizenship country has a tax treaty with US but not the country where I am working now (although I do have a permanent address here).
Line 5 ? Mailing address - I have my US address Line 6 ? US tax payer Identification number - I have my EIN Line 7 - None Line 8 - Reference number - does something need to be filled in here? Part II
9.a ? Again, do I enter the country of my citizenship or where I am working now? 9.b ? I have EIN 9.c ? Is this applicable for me or my corporation? 9.d ? Is this applicable for me or my corporation? 9.e ? Is this applicable for me or my corporation? Line 10 ? What would go in the "%" field? "rate of withholding on (specify type of income)" ? If ecommerce business, can I just put that in? "Explain the reasons?." ? What do I write in here?In line 10 - someone advised me to leave it blank?
