Document to put insurance policy "in trust"

I'm looking for a document to put a life policy in trust. I've done this before, and could conceivably hack a recent example around a bit and do it that way. However, I was wondering if these can be found off-the-peg: they must be fairly standard. Is there such a thing as a legal publisher that would do such a thing (I'd be happy to pay a nominal fee for a standard document)? I could approach a solicitor, but that seems a bit of overkill for something fairly simple that I could probably do for myself. The insurance company in question doesn't supply forms any more (& I also want to do forms for a couple of policies with different companies, so a generic form would be a bit easier). I'm doing a bit of IHT/PET spring-cleaning and found a couple of life policies that aren't written in trust. TIA Allan

Trust documents from different providers have different wordings and I believe it's to do with the way the policies are structured. You may find that one provider's form is not suitable for a policy of another.
