FSA Endowment ruling

The FSA have said that I do have a case against the company that sold me my endowment and have told them to make me an offer. I have been sent a form to fill in from the company asking all sorts details, many of witch I simply don't remember, such as the name of the endowment adviser? what were you earning at the time? what age were you hoping to retire at when you to out the endowment? etc etc. Having got this far I don't was to prejudice my case buy filling in one of these questions wrongly. It would be easy to guesstimate an answer accounting for the passage of time and get it wrong. I told that there is a formula that the company has to work to, is this the case? Is there an idiots guide to filling in these forms on the web? Is this the time to get some independent help?
