If one doesn't come automatically, yes. But then the IR will know that you need it, since you informed them within 3 months of becoming self-employed - didn't you??
Does it cause any complications being paid by PAYE and SE at the same time?
You just need to fill in both "Employment" and "Self-employment" sections of the SA form...
