Widow's Pensions

If a married man receiving a pension from an occupational scheme dies, his widow will normally continue to receive a (probably reduced) pension. Is it usual for this to cease if she remarries? If she starts to live with a man but unmarried?

I suspect that this will be down to the rules of the scheme, but I'm interested in whether there is a "standard" answer covering 80% of such schemes.
