The window to add / edit all paycheck items is now blank other than name, date, and account? I've tried reinstalling / fix errors download from the CD.

You haven't provided your Quicken version/release. Something like this was reported with one of the releases of Q2007. Some users worked around the problem by reverting to an earlier release (one reverted all the way back to R1, if I recall), others by upgrading to Q2008.
