Does anyone know how to open a BOS Business Current Account? There is a section on the form that simply states "cheque amount" and another section with "nominated account", but no mention of opening balance anywhere on the form or on their website. Is nominated account for paying the opening balance by bank transfer instead of using a cheque?

God knows the man filled the form in for me. :) Are you not meeting with someone there to go through all the stuff they can offer you?
In our case they had someone call us and we filled in the form "over the phone" - a printed copy being sent in the post for signing. If I were the OP I would call them (or ask for a call back).
It's new business rather than customer service so expect excellent service at this stage.
I saw them today. It's £500 upfront with 12 months free business banking.
And then free banking if you're 5k in credit? Any idea how much on foreign bank transfers? (will have to email the man later)
