Do NOT be taken in by this company!
They spent 30 minutes on the phone convincing my wife that they were legit. She does medical billing for a living, so she was extraordinarily wary of them. However, in the end they sold her. When the paperwork arrived, the following disclosure was included:

The plan provides discounts at certain health care providers for medical services. The plan does not make payments directly to the providers of medical services. The plan member is obligated to pay for all health care services, but will receive a discount from those health care providers who have contracted with the discount plan organization. - Pinnacle Choice provides savings to its members on healthcare services through a number of networks. In order to access these networks and the related discounts, member or member's dependents must pay the medical providers promptly. Specifically, payments on all medical bills are due and payable at the time of service. If payment plans are arranged with medical providers and the agreed upon payment is not made, the cost of service will revert back to the undiscounted fee.
- When necessary to re-price, Pinnacle Choice may provide network fee information to medical providers under this program and send a copy of the re-price to our member.
- Neither Pinnacle Choice, any of its affiliates, nor any network accessed shall be liable for any payment to a provider, accessed under the Pinnacle Choice program, or any refusal of participating providers to accept the network fee offered under this program. Pinnacle Choice, its affiliates or any network accessed is not an insurer, guarantor or underwriter of the responsibility or liability of Member for Member's or Member's dependents' medical care or any other goods or services provided to Member or Member's dependents.
- Information to access a directory of participating providers is provided to member on enrollment. The providers are subject to change without notice. Member may call the network's 800 number or Pinnacle Choice's customer service line for current provider information. There are also provider search functions available at

- Participating medical\lo providers are independent contractors and Pinnacle Choice, its affiliates and contracted networks, are not responsible for health care provided or the omission of the provision of health care by any provider. Pinnacle Choice does not practice medicine or in any manner interfere with or participate in the provider-patient relationship. All health care decisions are between the patient and the provider. The selection of a provider is the obligation and decision of the patient and is not based upon the credentialing of providers or any recommendation by Pinnacle Choice, its affiliates or its contracted networks.
- To add additional members of your household to your plan, simply call our CS department.
- Payments for the Pinnacle Choice program are due in advance or paid monthly through e-checks or credit card. Pinnacle Choice reserves the right to terminate any enrollment or deny eligibility in the program for lack of payment to Pinnacle Choice. Returned checks, insufficient funds or denial by the member's credit card company for payment of the periodic program fee is deemed evidence of non-payment by member. There will be a twenty-five (.00) charge to be reinstated in the program after such denial. To change your method of payment or frequency, simply call our CS department. Each program charges a one time, non-refundable enrollment fee of one hundred and thirty-five dollars (5.00), included with the first months payment.
- For annual payment plans, your membership will automatically renew on your anniversary date. We will charge your bank account or credit card on file. For installment payment plans, we will continuously charge your credit card or bank account until you choose to cancel. For members joining through a special promotion, rules and conditions of that particular promotion shall apply.
- Members may cancel at any time upon written notice and return of ID cards. Service issues should be brought to the attention of Pinnacle Choice or its representatives. Any an all complaints should be brought to the attention of a manager.
