Offline Database Sync Error/I.E. Script error

We receive these errors periodically on our POS stations:

"Run-time error '-2147217873 (80040e2f)': Violation of PRIMARY KEY constraint 'PK_Item'. Cannot insert duplicate key in object 'Item.'

We also use a custom html for the POS window, and we are getting an internet expolorer script error showing the URL as that file.

Any suggestions?

Thanks for the input Jeff. That seemed to help on one machine, but are still getting it on one of our registers regularly.

Let's say it's POS3. I connected to (local) and to what I named 'offlinePOS3'. Deleted it and created another one. After it's created I test the connection. Everything is good on that end.

Then, I do a Z report so I can sync. It starts to sync, but gets stuck on 'Item' then after awhile we get two errors simultaneously:

After clicking okay, it boots you out of POS.

I followed the SQL Connection guideline Jeff so kindly provided, but am stuck!
