Is there a way to automatically add a scanned item to the transaction? It seems silly to have to hit enter with each item. I'm hoping its just a setting I haven't found yet. I have enabled touchless transactions.
The carriage return is a setting on your scanner, not RMS. Check your scanner booklet and within it you should have a bunch of barcodes to scan that serve as programming triggers on your scanner.

You need to program your scanner to add an 'Enter' after each scan. Check your scanner docs. It should be relatively easy. You usually scan a programming barcode to put the scanner into program mode and then one to add the ENTER suffix and then one to end programming mode.

Jocelyn is correct about the scanner configuration. Also, you refer to "touchless transactions" so I'm assuming you have POS and not RMS. If this is the case, in addition to programming the scanner to include a carriage return, be sure to select the device from the Register Properties dialog. In most cases you should choose the Keyboard Wedge Scanner option. This is an important step for POS that isn't in RMS. If you do in fact have RMS, then... never mind ;)
