Matrix Items and Sub Descriptions Keep Going Out of Sync

We have had a situation where we created matirix items with 2 sub descriptions in RMS using the New Item Wizard initially. Later when we had to add items to a matrix we added them as individual items. Somehow the main description and the sub descriptions became massively out of sync. The main description might say blue and the sub description might say charchol. As a result we have wasted a huge amount of time correcting the database, re-printing and re-taggging everything, and retaking and re-entering the inventory. Most of the sub descriptions that were changed were added by New Item Wizard so human error is not possible. What could be causing this problem and what can be done to avoid it?
